About Internship Courses – CS 4100, CS 5100, CS 7100

The Approval Process

The student should complete the approval process at least a week before the 1st day of class.

  1. The student must complete the CS 4100 approval form, CS 5100 approval form, or the CS 7100 approval form. The student and internship supervisor (at your place of employment/internship) need to complete sections on the form.
  2. The student must obtain a copy of the offer letter printed on company letterhead.
  3. When forms are completed and signed, send all required documents in an email to Karen Hollensbe and request an appointment to meet with the department Chair. Please identify 2 options of days/times that might work for you.

The following documents must be in received by Karen Hollensbe in order to secure an appointment with the CS Chair:

    • Offer Letter on Company Letterhead
    • Completed and signed internship course approval form (for either CS4100, 5100 or 7100)
    • CPT form (top portion completed by student) – for international students only

  4. If the department chair approves your internship:
    • An override will be performed by the department to permit your registration for the appropriate section of CS 4100, 5100 or 7100.
    • Once the override is in the system, you will be notified that you are now able to register for the appropriate section of your Internship course.
    • International students work with ISSS to file their CPT.
